For a Homunculus to drop from a monster in Diablo 2: Resurrected, you must first defeat the monster that drops it. This will only occur at random throughout the course of the game. However, while it is impossible to guarantee that you will obtain a Homunclus, there are methods of increasing your chances of obtaining one, such as repeatedly playing the sections of the game where a monster is most likely to drop one or using a special item to increase your chances of receiving one.
It is not possible to obtain Homunculus on the Normal difficulty due to the difficulty level being too high. While the Palace Cellar on Nightmare difficulty might be a good place to start looking for one, don't expect to find one for a long time. According to the Nightmare difficulty, the most likely character to drop Homunculus is Baal, who is then followed by Andariel on the Hell difficulty and finally Mephisto on the Nightmare difficulty, according to the other two difficulties. There is a reasonable chance that it will be dropped by any of the bosses throughout the entire game on the Hell difficulty, as well as in Acts III, IV, and V of the Nightmare difficulty.
If you find boss battles to be too repetitive and would prefer to farm an entire Act rather than completing it in a single sitting, Act V on Nightmare difficulty is probably your best bet for finishing the game. This will almost certainly result in a significant slowdown; however, you never know, you might get lucky and avoid a significant slowdown altogether!
It is the best shield for Necromancers when it comes to pure physical defense, and it also provides a significant boost to all resistances when used in conjunction with other D2R items for sale to maximize its effectiveness.
